We are seeking an experienced Hospice RN Case Manager to provide compassionate, patient-centered care to individuals receiving hospice services throughout the East Bay Area.
The Hospice Nurse is responsible for admitting patients to hospice services, assessing patient and family needs, developing individualized plans of care, and coordinating services with a multidisciplinary team to ensure quality end-of-life care.
Required Qualifications
- Active and valid Registered Nurse (RN) license.
- Minimum of 1 year of hospice nursing experience required.
- Experience with hospice patient assessments, care planning, and case management.
- Strong clinical assessment, documentation, and care coordination skills.
- Ability to work independently and collaboratively with a multidisciplinary team.
