🕗 Shift: Days, 8:30 AM–5:00 PM (flexible based on patient availability)
⏱️ Hours: 40 hours per week
🏥 Setting: Hospice / Home Health
💻 EMR: Homecare Homebase (HCHB)
📆 Weekends: One weekend per month
🎄 Holiday Coverage: Required
📞 Call: No call
🗺️ Territory: Oakland, Alameda, San Leandro, Castro Valley, Hayward, Fremont, Milpitas, and surrounding cities
Requirements
🩺 Active California RN license
⏳ Minimum 3 years of Hospice RN experience
📜 Current BLS/BCLS certification
📝 Experience admitting hospice patients and conducting comprehensive patient assessments
💜 Ability to develop individualized plans of care and coordinate with multidisciplinary teams
🤝 Strong patient and family communication, documentation, and follow-up skills
