Job Title: Registered Nurse (RN) – Hospice Case Manager
Facility Type/Setting: Home Health / Hospice
Facility/Location: Buncombe County, NC
Shift & Schedule: 6:45 PM–7:15 AM
Weekly Hours: 36 Guaranteed Hours
Contract Length: 13 Weeks
Requirements
- Active RN license
- Hospice experience required
- Familiarity with EMR documentation
- Active driver's license
- Understanding of documentation required for hospice certifications
- CPR certification
- Home health experience preferred
- Hospice case management experience preferred
Responsibilities
- Provide individualized hospice and palliative nursing care to terminally ill patients.
- Assess patients and manage pain and other care needs.
- Develop and deliver patient-specific care plans in collaboration with the hospice interdisciplinary team.
- Educate patients' families and caregivers.
- Maintain required hospice certification documentation.
