Job Title: Home Health Care Registered Nurse (RN)
Facility Type/Setting: Home Health
Shift & Schedule: Day Shift | 8-hour shifts
Weekly Hours: 40 hours
Contract Length: 13 weeks
Requirements:
- Active RN license.
- Ability to provide skilled nursing care independently in patients' homes.
- Ability to manage a patient caseload and collaborate with other care disciplines.
Responsibilities:
- Plan, organize, and direct home care services based on patient needs.
- Administer skilled nursing care in patients' homes according to physician orders and the established plan of care.
- Manage patient caseloads effectively and efficiently.
- Collaborate with other healthcare disciplines to achieve patient care goals.
- Educate and provide emotional support to patients and their families.
- Serve as a clinical resource for patients, family members, and care team members.
- Work under the direction and supervision of the Director of Clinical Services/Nursing Supervisor.
