Job Description
The RN Care Manager is responsible for planning, organizing, and directing home care services to meet patients' healthcare needs in their homes.
The role involves managing an assigned patient caseload efficiently while providing independent nursing care and collaborating with other healthcare disciplines to ensure patient care goals are achieved in accordance with individualized care plans.
The RN Care Manager also serves as a clinical resource and support system for patients, family members, and other members of the healthcare team.
Requirements
- Day shift required.
- EPIC experience is mandatory.
- Mileage reimbursement provided in addition to the bill rate (BR).
Education & Work Experience
- Bachelor's Degree in Nursing (BSN) – Preferred.
- Experience in an acute care facility or home health setting – Preferred.
Licenses & Certifications
- Active Registered Nurse (RN) license in the state of practice – Required.
- Current CPR or Basic Life Support (BLS, HS-BLS, or RQI-BLS) certification from an approved vendor, per AH policy – Required.
- Valid Driver's License – Required.
- Must be at least 21 years of age.
- Current automobile insurance – Required.
- Reliable personal vehicle – Required.
