Patient Care Manager
This position is for a Patient Care Manager in a home health office, primarily working in an office-based environment. The role is responsible for overseeing various operational and clinical tasks related to patient care and management.
Key Responsibilities
- Manage the office dashboard, reviewing orders and maintaining contact with physicians and clinicians.
- Facilitate new admissions and assist with scheduling as needed.
- Occasionally assist with patient visits outside of the office.
Qualifications
- Minimum of 2 years of nursing experience, including 1 year as a General Registered Nurse (RN) and 1 year in home health care.
- New graduates are welcome to apply.
- Must possess a valid South Carolina RN License.
- Basic Life Support (BLS) certification is required.
Clinical Requirements
- Experience with Homecare Homebase (HCHB) is required until October 1, 2026; Epic experience will be required after this date.
- If the candidate lacks HCHB experience, completion of an online HCHB course prior to starting is mandatory, with the cost to be borne by the candidate.
Work Schedule and Expectations
- This is a day position with a 5x8-hour work schedule (08:00 - 16:30).
- Maximum request for time off (RTO) is 7 days.
- Floating to other areas within scope of practice may be required.
- Some on-call responsibilities may be expected, potentially once per week.
Compensation and Compliance
- Mileage will be reimbursed at the IRS standard rate, tracked through HCHB.
- Holidays: Expected to work on major holidays based on the assignment schedule. Details provided upon hiring.
- Full compliance with all requirements is necessary for timely onboarding; delays may push start dates back by two weeks.
Candidates will undergo a clinical screening to determine the fit for the role before final approval by management. Both traveling and local candidates are welcome to apply at the same rate.
