Pride Health is hiring a Home Health Registered Nurse (RN) to support our client's healthcare facility in Orangeburg, South Carolina.
Job Details:
Schedule: Monday-Friday, until route/shift is complete
Location: Orangeburg, SC 29118
Float / Coverage Area: Yes, local travel within assigned service area required
Job Type: Local and Travel Contract
Contract Length: 8 Weeks (320 Total Hours)
Mileage Reimbursement Rate (if applicable): Yes
Pay offered is based on experience, expertise, credentialing, and education.
Duties:
- Conduct comprehensive home health patient assessments and complete OASIS documentation to develop individualized care plans.
- Deliver skilled nursing care in patients' homes, including medication management, wound care, IV therapy, catheter care, and disease management.
- Coordinate care with physicians, interdisciplinary teams, patients, families, and community resources to ensure continuity of care.
- Educate patients and caregivers on medications, home safety, disease management, pain control, and self-care practices.
- Maintain accurate clinical documentation in Homecare Homebase (HCHB) and ensure compliance with regulatory and payer requirements.
Requirements:
Education:
- Graduate of an accredited Associate Degree in Nursing (ADN) or Nursing Diploma Program required.
- Bachelor of Science in Nursing (BSN) preferred.
License(s):
- Current South Carolina Registered Nurse (RN) License required.
- Valid Driver's License required.
Certification(s):
- Current CPR Certification required.
- CHPN Certification preferred.
Years of Experience:
- Minimum 1 year of RN experience in a clinical care setting required.
- Minimum 2 years of Home Health experience required.
- At least 1 year of OASIS experience required.
- At least 1 year of Homecare Homebase (HCHB) experience required.
