Patient Care Manager – Home Health (RN)
- Location: Office-based, South Carolina
- Schedule: Monday–Friday
- Experience: 2 years minimum (1 year General RN + 1 year Home Health); first-timers accepted
- Requirements: BLS, SC RN License
- Systems: HCHB experience/course required; Epic required after 10/1/2026
- Responsibilities: Review orders/dashboard, coordinate with physicians and clinicians, manage new admissions, and possibly assist with scheduling and patient visits
- Call: Possibly 1 shift/week
- Floating: Within facility/scope as needed
RTO: Maximum 7 days
