2-year acute medical Care Manager experience performing Care Coordination recent experience is a must, within the last 3 months
Required Qualifications
- Minimum 2 years of recent acute medical Care Manager experience performing care coordination
- Recent acute care hospital experience is required
- Previous travel nursing experience is required
- Strong experience with care coordination and discharge planning
- Experience assessing patient needs and developing plans of care
- Ability to independently arrange DME, SNF placement, and ambulance transportation
- Epic experience is required
Required Certifications & Licenses
- Active California RN License required
- Additional certifications may be required based on facility compliance
