Travel RN Case Manager (Acute Care)
Providence Mission Viejo, 27700 Medical Center Rd, Mission Viejo, CA 92691
- Start / Duration: 10/26/2026, 13 weeks
- Schedule: Days, 5x8 hrs (8:00 AM to 4:30 PM)
- Weekends: Variable 5-day workweek (department runs 7 days); days off may not be back-to-back
- Ratio / Census: 1:25, up to 25 patients (adults and geriatrics)
- Float: Laguna facility
- Orientation: 40 hours
- Time off: 1 pre-approved request
- Pay: $67.00/hrs.
Must-Have Skills:
- Active CA RN license (a pending license is accepted but must be active by the start date)
- Previous travel experience (hard stop, no first timers)
- Epic charting (hard stop)
- 2+ years of acute medical case management and care coordination, with recent experience within the last 3 months
- Assessment, discharge planning, and treatment plan development
- Arranging DME, SNF, and ambulance transport independently
- Experience with HMOs and IPAs
- Running family meetings
- Knowledge of advance directives, POLST, protective services, adoption/foster care, substance abuse treatment, homelessness, and mental health crisis
