Required Experience
- 2+ years of recent acute-care RN Case Management experience
- Recent Care Coordination experience is required and must be within the last 3 months
- Strong experience with:
- Patient assessment
- Care coordination
- Discharge planning
- Treatment planning
- Family meetings
- HMO/IPA coordination
- DME, SNF and ambulance arrangements
- Knowledge of resources related to mental health, homelessness, substance abuse, advance directives/POLST, protective services, adoption/foster care, etc.
- Must be able to independently arrange DME, SNF and ambulance transportation.
