Requirements
- Minimum 2 years of recent acute medical Care Manager experience
- Recent Care Coordination experience within the last 3 months
- Epic experience required
- Active CA RN License required by start
- Previous travel RN experience required
- Strong experience with:
- Care Coordination
- Discharge Planning
- Patient Assessments
- Treatment Planning
- Family Meetings
- DME, SNF & ambulance arrangements
- HMO/IPA coordination
- Complex psychosocial/discharge needs
- Must be able to independently arrange DME, SNF placement and transportation
- Trauma Level I/II experience preferred
