RN – Home Health | Pennsylvania
Shift: Days – 5x8 (8 AM–4:30 PM)
Hours: 40 hrs/week
Experience: 2 years Home Health & Acute Care
First-Time Travelers: Accepted
Certifications: BLS, OASIS
Requirements: Skilled Medicare-certified Home Health experience required (private home care not accepted). Medicare OASIS knowledge and experience mandatory. Matrix EMR highly preferred.
Clinical Skills: Patient assessments, Start of Care, wound care/VAC, IV infusions, port/PICC care, catheter management, medication management, phlebotomy and case management.
Patient Diagnoses: CHF, Hypertension, Diabetes, Total Joint Replacement and Oncology.
Schedule: Monday–Friday, 8 AM–4:30 PM. No weekends or mandatory call. requirements section
