Case Manager Registered Nurse (RN)
Position Overview
The Case Manager Registered Nurse plays a crucial role in coordinating patient care and managing healthcare services for individuals with a focus on quality outcomes. This position requires a professional individual who is committed to delivering compassionate care while efficiently navigating the healthcare system.
Schedule
- Shifts: 4x10 or 5x8 days available
- Weekends: Required; every other weekend or four weekend days within a four-week schedule
- Guaranteed Hours: 72 hours per two weeks; traveler may be called off for one 8-hour shift per two weeks
Key Responsibilities
- Develop and implement individualized patient care plans
- Collaborate with healthcare providers and support staff to ensure comprehensive patient care
- Facilitate communication between patients, family members, and medical professionals
- Conduct patient assessments to identify needs and resources
- Monitor patient progress and adjust care plans as necessary
- Maintain accurate and timely documentation using Epic electronic health record system
Qualifications
- Active Registered Nurse (RN) license in Alaska
- Minimum of 1 year of experience in a clinical setting, preferably in acute care
- Basic Life Support (BLS) certification required
- ACM Certification preferred
- Epic experience is required
Additional Information
- No block schedule arrangements available
- Local candidates not eligible; candidates' permanent address must be at least 50 miles from the facility
- One time off request per 13-week assignment, not exceeding five days; holiday time off is not available for auto offer
