Case Manager Registered Nurse (RN)
We are seeking a dedicated and experienced Case Manager Registered Nurse (RN) to join our team. This position involves coordinating and managing patient care to ensure optimal health outcomes.
Job Details
- Shift: Day shifts available, 4x10 or 5x8-hour schedules
- Contract Length: 13 or 26 weeks
- Guaranteed Hours: 72 hours per 2 weeks; potential for call-offs
Responsibilities
- Coordinate patient care and services for acute care settings
- Conduct assessments to identify patient needs and develop care plans
- Collaborate with interdisciplinary teams to ensure comprehensive care delivery
- Utilize Epic for charting and documentation
- Float within the scope of practice to meet facility needs as required
- Work weekends, with a schedule that includes every other weekend or 4 weekend days within a 4-week cycle
Qualifications
- Education: Valid Registered Nurse (RN) license in Alaska is required
- Experience: Minimum of 1 year of experience in case management or acute care
- Certifications: BLS certification required; ACM certification preferred
- Skills: Proficiency in Epic and experience in an acute care setting are essential
Additional Information
- Local candidates are not accepted; must reside at least 50 miles from the facility
- Only 1 time-off request of 5 days or less is permitted during the 13-week assignment
- No holiday leave is allowed for automatic offers
- Parking is free for staff
