Job description
This position will support the Emergency Department. The Palliative Care Nurse Navigator is a specialized registered nurse who guides patients and their families through serious or life-limiting illnesses. They coordinate care, triage referrals, facilitate goals-of-care discussions, provide education regarding disease processes, and help connect patients to community resources to improve their quality of life.
Minimum Education
- Graduate of an approved school of nursing, B.S.N. preferred.
Minimum Work Experience
- 3 years of professional nursing experience.
- 2 years of palliative or hospice care preferred.
Required Licenses/Certifications
- Current Vermont RN license.
- BLS Certification through American Heart Associations.
- Certified Hospice & Palliative Nurse (CHPN) or equivalent certification*
*Or obtained within 2 years of hire.

- Demonstrates safe knowledge of nursing practice skills at all times based on theory and consistent with policy and procedure.
- Demonstrates specialty knowledge and practice of nursing.
- Excellent interpersonal, written, and verbal communication skills.
- Ability to effectively communicate and relate information to individuals of varying ages from diverse socio-economic backgrounds.
- In depth knowledge of utilization management criteria, complex medical and surgical patients, reimbursement and regulatory requirements including CMS guidelines.
- Demonstrated strong attention to detail.
- Demonstrated strong knowledge of basic computer skills.
Pay Range = $38.91 - $56.68
