Job description

This position will support the Emergency Department.  The Palliative Care Nurse Navigator is a specialized registered nurse who guides patients and their families through serious or life-limiting illnesses. They coordinate care, triage referrals, facilitate goals-of-care discussions, provide education regarding disease processes, and help connect patients to community resources to improve their quality of life.

Minimum Education

  • Graduate of an approved school of nursing, B.S.N. preferred.

Minimum Work Experience

  • 3 years of professional nursing experience.
  • 2 years of palliative or hospice care preferred.

Required Licenses/Certifications

  • Current Vermont RN license.
  • BLS Certification through American Heart Associations.
  • Certified Hospice & Palliative Nurse (CHPN) or equivalent certification*  

*Or obtained within 2 years of hire. 

Required Skills, Knowledge, and Abilities

  • Demonstrates safe knowledge of nursing practice skills at all times based on theory and consistent with policy and procedure.
  • Demonstrates specialty knowledge and practice of nursing.
  • Excellent interpersonal, written, and verbal communication skills.
  • Ability to effectively communicate and relate information to individuals of varying ages from diverse socio-economic backgrounds.
  • In depth knowledge of utilization management criteria, complex medical and surgical patients, reimbursement and regulatory requirements including CMS guidelines.
  • Demonstrated strong attention to detail.
  • Demonstrated strong knowledge of basic computer skills.

Pay Range = $38.91 - $56.68
