POSITION SUMMARY: RN Case Manager
POSITION DUTIES: Discharge Planning
MINIMUM REQUIRED QUALIFICATIONS: RN, 5+ years' experience
PREFERRED QUALIFICATIONS: BSN, RN
LENGTH OF ASSIGNMENT: 13 weeks
SHIFT / HOURS PER WEEK: M-F 8am-4:30pm
SYSTEMS: EPIC
