Job Description:
Seeking experienced Care Coordinator / Case Management RNs to manage inpatient care coordination, discharge planning, and safe patient transitions at a leading Philadelphia hospital.
Candidates must have strong hospital-based case management experience and the ability to independently manage a daily caseload of 18–25 patients.
Must-Have Qualifications:
- Active RN license required; Pennsylvania RN license required upon hire.
- Minimum 5 years of professional experience.
- Prior inpatient hospital case management experience required.
- Strong discharge planning and care coordination experience.
- Bachelor's degree required per the hiring requirements.
- Experience collaborating with physicians, nursing, social work, and interdisciplinary teams.
- Ability to independently manage 18–25 patients per day.
