Job Requirements
- Registered Nurse (RN) license required
- Associate degree in nursing or higher
- Recent home health experience strongly preferred
- Experience with HCHB is a plus
- Strong communication, time management, and documentation skills
- Ability to work independently while covering a defined territory
- Reliable transportation for travel between patient visits; mileage reimbursement is provided between visits
Responsibilities
- Serve as an RN case manager for a designated territory
- Complete skilled routine visits, recertifications, discharges, and start-of-care/resumption-of-care visits
- Deliver compassionate, 100% patient-focused care in the home environment
- Coordinate care and collaborate with interdisciplinary team members to support positive patient outcomes
- Manage a patient caseload with productivity expectations while maintaining quality care
- Cover assigned patients and assist teammates as needed on days off
- Follow clinical standards, documentation requirements, and care plans accurately and in a timely manner
- Participate in rotating on-call and weekend coverage as scheduled
