Med Ninjas is seeking a travel nurse RN Long Term Care for a travel nursing job in Gallup, New Mexico.

Job Summary (List Format):
- Provide comprehensive nursing care to residents in a long-term care/skilled nursing facility.
- Conduct initial, ongoing, and focused resident assessments; monitor and report changes in condition.
- Develop, implement, and update individualized care plans based on residents’ needs.
- Administer medications, IV therapy, injections, treatments, and other therapies per physician orders.
- Monitor and manage residents with chronic medical conditions (e.g., cardiovascular, respiratory, diabetes, neurological).
- Perform wound assessments and care, including dressing changes and pressure injury prevention.
- Monitor vital signs, blood glucose, intake/output, and other clinical indicators.
- Recognize and report changes in residents’ conditions promptly to physicians or healthcare providers.
- Respond to medical emergencies, initiate CPR, and provide emergency interventions when needed.
- Maintain accurate and timely documentation in EMR/EHR systems.
- Coordinate care with physicians, nurse practitioners, CNAs, therapists, pharmacists, and other healthcare team members.
- Supervise and provide direction to CNAs and other nursing staff.
- Complete medication reconciliation and monitor for medication effectiveness and adverse reactions.
- Assist with admissions, transfers, discharges, and comprehensive resident handoffs.
- Educate and support residents and their families about medications, treatment plans, and ongoing care.
- Follow infection-control, isolation, fall-prevention, and patient-safety protocols.
- Ensure compliance with facility policies, state regulations, and CMS/long-term care standards.
- Assist residents with activities of daily living, promoting comfort, dignity, independence, and quality of life.
- Monitor residents for falls, pressure injuries, infections, dehydration, and other complications.
- Communicate effectively with residents, families, physicians, and interdisciplinary care teams to ensure coordinated care.
Med Ninjas Job ID #15285. Pay package is based on 12 hour shifts and 36 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
