Registered Nurse (RN) - Hospice
Position Information
- Role: RN
- Specialty: Hospice
- Shift: 0800-1700
- Weekly Guaranteed Hours: 40
- Weekend Hours: Every other weekend
- On-call Rotation: Overnight and weekend on-call required
- Expected Productivity: 25 visits per week
- Experience Required: Minimum of 1 year in home health or hospice
Job Responsibilities
- Provide comprehensive patient care and support in hospice settings.
- Conduct routine visits, manage new admissions, and follow up on post-fall incidents.
- Utilize electronic medical records (EMR) effectively for documentation and reporting.
- Case Manage 14-18 patients based on acuity.
- Perform patient assessments and implement care plans tailored to individual needs.
Education & Certifications Required
- Certifications: BLS (Basic Life Support)
- Education Modules: As per hospice protocol
Unit Information
- Patient Ratios: Managing 5-6 visits per day.
- Equipment Provided: Agency laptop, iPhone, nursing supply bin, nursing bag with vital signs equipment.
- EMR System: EPIC
Hiring Profile
- Must Have Skills: Proficiency in Epic EMR
- Preferred Skills: Experience with PICC lines, dressing changes, trachs, port access, ostomies, chest tubes, Pleurex drains, lab draws, and urinary analysis.
- Patient Types: Adult medical-surgical, oncology, cardiac, neurological, respiratory, wound care, gastrointestinal, urological.
- Personality Fit: Strong collaborator, good communication skills, open to feedback for continuous improvement.
Dress Code
- Scrub Color: Business casual or scrubs (any color)
- Unacceptable Attire: No jeans, open-toed shoes, or clothing with holes
Additional Information
- Successful case management and self-scheduling of patients are essential.
- Utilization of in-home infusion pumps; however, no ventilators or invasive monitoring required.
