Home Health RN | Monday–Friday
- Schedule: 8:00 a.m.–4:00 p.m.
- Provide skilled nursing care to patients of all ages in their homes.
- Assess physical, psychosocial, environmental, and pain-management needs, including stable and emergent situations.
- Develop, implement, and evaluate individualized plans of care.
- Coordinate services with patients, families, physicians, referring agencies, and the interdisciplinary team.
- Educate patients and caregivers on medications, diet, home safety, infection prevention, and self-care.
- Maintain timely, accurate documentation and monitor payer requirements.
- Supervise and provide clinical guidance to home health aides, STNAs, and LVNs.
Arrange referrals and participate in interdisciplinary care meetings.
