Job Description – RN Home Health/Hospice
Position: Registered Nurse (RN) – Home Health/Hospice
Location: Durham County, NC
Shift: Day Shift – 5x8 Hours (8:00 AM – 5:00 PM)
Schedule: Monday – Friday
Job Responsibilities
- Provide skilled nursing care to patients in their homes throughout Durham County.
- Assist with managing patient caseloads and increased referral volumes.
- Conduct patient assessments and monitor health conditions.
- Develop, implement, and update individualized patient care plans.
- Coordinate patient care with physicians, healthcare professionals, and interdisciplinary teams.
- Maintain accurate and timely patient documentation using the EPIC charting system.
- Support patients and families with appropriate home health and hospice care services.
Requirements
- Minimum 2 years of RN experience.
- At least 1 year of recent Home Health/Hospice field experience required.
- EPIC charting experience required.
- First-time travelers are accepted.
- Local candidates are accepted.
Schedule & Additional Details
- Start Date: November 16, 2026 (soonest) or November 30, 2026 (latest).
- Monday–Friday, 8:00 AM–5:00 PM.
- No on-call requirements.
- No weekend requirements.
- Coverage throughout Durham County, NC.
- Additional RN support needed due to increased referrals and caseloads.
