Case Manager
Job Description
The Case Manager is responsible for assessing, evaluating, and implementing effective discharge planning for patients. This role includes coordinating services aligned with the appropriate level of care and ensuring that treatment plans are initiated, evaluated, and re-evaluated daily. The Case Manager will monitor patient progress and identify barriers to discharge while adhering to Medicare regulatory guidelines.
Key Responsibilities
- Assess and evaluate discharge plans for patients.
- Coordinate services to match the patient’s appropriate level of care.
- Ensure daily initiation, evaluation, and re-evaluation of treatment plans.
- Monitor patient progress daily and address any lack of progress.
- Track length of stay and identify barriers affecting discharge.
- Comply with Medicare regulatory guidelines regarding patient choice, IMM notices, and MOONS.
Qualifications
- Minimum of 3 years of experience in case management.
- First-time travelers are not accepted.
- Clinical screening required to assess fit within guidelines.
Schedule
- Monday to Friday
- No weekend shifts
- No on-call requirements
Time Off Requests
Maximum 7 days of time off is allowed; requests should be discussed during the interview process. Note that no time off requests will be accepted during September.
Holiday Requirements
If one major holiday falls within the assignment period, the traveler is expected to work it. For two major holidays, working both is required. For three major holidays, two must be worked. Major holidays include:
- Memorial Day
- Independence Day
- Labor Day
- Thanksgiving/Black Friday
- Christmas Eve/Christmas Day
- New Year's Eve/New Year's Day
Compliance
Full compliance is required to start on time. Non-compliance may result in a two-week pushback due to the bi-weekly orientation schedule.
Travelers and Locals
Local candidates are accepted at the same rate as travelers.
