Registered Nurse (RN) - Case Manager
Job Description
We are seeking a dedicated RN Care Manager to provide comfort, compassion, and high-quality care to patients and families in need. The primary focus of this role is to manage patient care effectively, ensuring that comprehensive services and support are delivered in a timely manner.
Responsibilities
- Manage patient care within the home and community settings.
- Provide symptom management, crisis intervention, and family education.
- Maintain effective communication and rapport with patients and their families.
- Work collaboratively within an interdisciplinary team to achieve optimal patient outcomes.
- Travel to patients' residences, long-term care facilities, and hospitals is required as part of the role.
Working Conditions
This position primarily involves working in patients' homes, necessitating travel throughout the designated service area. Overnight travel for continuing education may also be required. There is potential exposure to infections and blood-borne pathogens in this role.
Qualifications
- Graduate from an accredited school of nursing.
- Current nursing license to practice in Virginia as verified via Nursys.
- A minimum of two years’ experience in acute care, home health, hospice, medical/surgical, or geriatric nursing.
- Valid Virginia driver’s license with a driving record that meets current policy standards.
- Basic Life Support (BLS) certification from the American Heart Association (AHA).
