**Key Responsibilities:**
- Provide case management services to patients, ensuring continuity of care.
- Collaborate with healthcare teams to develop and implement patient care plans.
- Conduct assessments and evaluations to determine patient needs.
- Facilitate communication between patients, families, and healthcare providers.
**Requirements:**
- 2 years of Case Manager experience required.
- Current RN License required; Nursys must be on profile.
- BLS certification required.
- Degree accreditation or 5 years of experience required.
- Willingness to float to other sister campuses as needed.
**Additional Information:**
- All RTO must be submitted with application; no approvals after submission.
- Demographic information (Address, email, full DOB, full SS, RN license number) must be loaded at the time of submission.
- Culture is very important; travelers are expected to integrate with core staff.
- Hard document deadline is the Monday prior to start; no exceptions for pending documents.
