Job Opportunity – Registered Nurse, Case Manager – Mission Hills, CA 91345
Start Date: October 12, 2026
Location: Mission Hills, California
Schedule: Day shift, 5x8-hour shifts (08:00 - 16:30), weekend rotation required to meet department needs. Patient ratio 1:20. No floating required.
Highlights:
- Six-month travel assignment at an acute care facility through April 2027
- Comprehensive 40-hour orientation with Epic charting system training
- Work with interdisciplinary support including social services and transportation teams
- Serve diverse patient populations including adolescents, adults, and geriatrics
- Business attire or scrubs accepted
- Up to 7 days of requested time off available
Requirements:
- Active California RN license (pending licenses accepted if active by start date)
- Minimum 2 years of acute care hospital Case Manager experience performing care coordination and discharge planning
- Hospital experience is required; behavioral health-only experience does not qualify
- Proficiency with Epic charting system required
- Experience with InterQual criteria, DRG, CMS guidelines, and plan of care development
- Competency in arranging DME, SNF placement, and ambulance transport independently
- Knowledge of HMOs, IPAs, and experience with complex discharge planning scenarios
- Proficient in Microsoft applications and computerized documentation
- Two professional references covering one year within the last three years
- Commitment to work 3 of 5 major holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
