Facility: Charter House, 211 Second Street N.W.
Shift: Nights | 6:00 PM – 6:00 AM
Hours: 36 hours/week
RN Preferred; LPN candidates will also be considered
Job Summary:
Responsible for coordinating and providing nursing care, patient/family education, care transitions, and supporting overall patient needs in an assisted living and short-term rehab setting.
Requirements:
- Assisted Living and Short-Term Rehab experience required
- Long-Term Care experience preferred
- Current RN/LPN license
- BLS/BCLS certification required
- Strong communication, leadership, mentoring, and patient-care skills
- Ability to work independently and coordinate patient care effectively
