Job Description
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our home health team. The ideal candidate will provide exceptional nursing care to patients in their homes, ensuring a comfortable and supportive environment for recovery and well-being.
Key Responsibilities
- Deliver direct nursing care to patients in their homes, adhering to established care plans.
- Monitor patient health and progress, adjusting care as necessary in collaboration with healthcare professionals.
- Educate patients and their families on managing illnesses and medications.
- Document patient interactions and treatments accurately in compliance with healthcare standards.
- Participate in patient assessments and contribute to individualized care plans.
- Provide emotional support to patients and families, fostering a positive care experience.
Qualifications
- Active Registered Nurse (RN) license in the state of practice.
- Minimum of 2 years of nursing experience, preferably in home health or community-based settings.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Current Basic Life Support (BLS) certification.
Working Conditions
This position requires travel to patient homes and may involve variable hours depending on patient needs. A valid driver's license and reliable transportation are essential.
