Travel Home Health Registered Nurse (RN)
Assignment Details
- Location: Barre, VT
- Contract Length: 20 Weeks
- Schedule: Monday–Friday, Days
- Guaranteed Hours: 36 hours per week
- Potential Hours: Up to 40 hours per week
Responsibilities
- Provide skilled nursing care to patients in their homes according to physician orders and individualized care plans.
- Perform comprehensive patient assessments and develop, implement, and evaluate care plans.
- Serve as the primary Case Manager when assigned, coordinating care with physicians, therapists, social workers, and other healthcare professionals.
- Educate patients and caregivers on disease management, medications, and home care strategies.
- Maintain accurate and timely clinical documentation.
Qualifications
- Active Vermont RN license (or eligibility to obtain prior to start)
- Current BLS certification
- Recent Home Health nursing experience required
