Job Description
The Registered Nurse Case Manager provides comprehensive, patient-centered nursing care in an outpatient setting, supporting individuals and families throughout their healthcare journey. This role involves the assessment, planning, implementation, and evaluation of care, while also administering medications, performing clinical procedures, and providing patient education and care coordination in collaboration with other healthcare providers and interdisciplinary teams.
Key Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Implement and evaluate nursing interventions based on patient responses and clinical circumstances.
- Administer medications and treatments accurately and safely in accordance with provider orders and policies.
- Recognize changes in patient conditions and initiate appropriate interventions or escalate care when necessary.
- Deliver culturally responsive and patient-centered care while providing education related to care, medications, and discharge needs.
Nursing Judgment & Clinical Decision-Making
- Apply critical thinking to perform and interpret assessments, responding to significant changes in patient conditions.
- Exercise independent nursing judgment in dictating appropriate nursing actions and determining when to consult with licensed independent providers.
- Develop, implement, evaluate, and revise individualized nursing care plans tailored to patient needs and preferences.
Care Coordination & Documentation
- Collaborate with providers and interdisciplinary team members to ensure coordinated care.
- Facilitate referrals and follow-up care, ensuring continuity of services for patients.
- Document assessments, interventions, and outcomes accurately and timely in electronic health records.
- Advocate for patients in navigating the healthcare system.
Quality, Safety & Compliance
- Adhere to regulatory, safety, and infection prevention requirements while practicing within the nursing scope of practice.
- Participate in quality and performance improvement initiatives.
- Maintain necessary competencies and certifications.
- Report adverse events and unsafe conditions as per established policies.
Teamwork & Professional Practice
- Demonstrate professionalism, accountability, and flexibility in all interactions.
- Provide guidance and delegation to ancillary staff as appropriate.
- Support the orientation, mentoring, and education of new staff, students, and peers.
Additional Information
Holiday coverage may be required. Must possess an active nursing license in the state of practice at the time of application.
