Schedule: 5x7.5-hour shifts | 37.5 hours/week
 

Requirements:

• Minimum 3 years of experience in Acute Care, Med/Surg, ICU, ED, or Community/Home Care nursing.
• Bachelor's Degree in Nursing (BSN) required.
• Active New York State RN license.
• BLS and ACLS required.
• Certified RN Case Manager required.
• PRI certification required.
• Knowledge of Milliman Care Guidelines and CMS post-acute placement guidelines.
• Epic experience and PRI proficiency required for the listed duties.

Responsibilities:

• Conduct patient assessments and identify barriers to safe discharge and self-care.
• Review medical necessity, severity of illness, and intensity of services.
• Coordinate with physicians, social workers, and interdisciplinary teams.
• Manage insurance reviews and documentation for admissions and continued stays.
• Coordinate discharge planning, SNF/rehab placements, home care, IV infusion, and wound VAC referrals.
• Document case management activities in Epic and coordinate referrals through CarePort.
• Help prevent delays, reduce readmissions, and ensure safe, timely discharges.
