UNIT: Home Health - Aroostook County CARE MANAGER
Manager Interview
Certs required: bls, vehicle, driver's license, auto insurance
Experience: 1 year. Cardiopulmonary assessment, Wound Care, Foley Catheter Management: male and female, Venipuncture/Phlebotomy, IV management, Ostomy Management
Staple removal, Pleural Catheter Management, Neurological Assessment.
Diabetes Management
CHF/COPD Management
Patient Types: S/P Orthopedic surgery, CHF/COPD disease management, wound care, neurological diagnosis, genitourinary conditions, Diabetes management, oncology, IV antibiotic teaching, medication management, venipuncture/phlebotomy, foley catheter management, ostomy management
RATIOS: 1 RN: 5 patient visits per day, case managing 30 patients
Scheduling:
Weekend rotation: Every 4 to 6 weeks
On call: 1 to 2 x month, 5 PM to 8 AM
Holiday rotation
Interviewing Manager can approve Time Off and Onsite Manager can approve time off throughout the assignment.
Shift times: 8 AM to 5 PM
Schedule cycle: (5) 8 hour days.
