Case Manager
Job Summary
The Case Manager is responsible for evaluating, assessing, and driving the discharge plan for patients. This role involves coordinating services to ensure patients receive care at an appropriate level and overseeing the daily initiation, evaluation, and re-evaluation of treatment plans.
Key Responsibilities
- Assess and evaluate patient needs to develop effective discharge plans.
- Coordinate services to align with the patient’s appropriate level of care.
- Ensure treatment plans are initiated, evaluated, and re-evaluated consistently each day.
- Monitor patient progress daily and identify barriers to discharge.
- Adhere to Medicare regulatory guidelines regarding patient choice, Notice of Medicare Non-Coverage (IMM notices), and Medicare Outpatient Observation Notice (MOON).
Qualifications
- Minimum of 3 years of case management experience required.
- Unit does not accept first-time travelers.
Schedule
This position operates on a Monday to Friday schedule, with no weekend or call requirements. Some floating within the facility may be required, as necessary and within the scope of practice.
Additional Information
- Maximum 7 days of requested time off (RTO), with no requests during September.
- Travelers are expected to work one major holiday if it falls within the assignment period, two holidays if two major holidays fall within the period, and two holidays if three major holidays fall within the period.
- Full compliance is required to start on time; non-compliance may result in delays.
- Local candidates are accepted at the same rate.
- Number of non-billable orientation hours is determined based on contract length.
