Pay Rate: $1800.00 - $2500.00
POSITION SUMMARY – RN Case Manager - Hospice
POSITION DUTIES – Assesses patients' physical, emotional, and psychosocial needs; develops and coordinates individualized end-of-life care plans; manages symptoms and pain control; educates patients and families; collaborates with interdisciplinary hospice team members; monitors care outcomes; and provides ongoing support to ensure comfort, dignity, and quality of life in the home setting. Productivity is 25 visits per week, looking for experienced with admissions.
MINIMUM REQUIRED QUALIFICATIONS –
- ME/compact RN license
- BLS
- 2 years recent home health/hospice case management experience
PREFERRED QUALIFICATIONS – Skilled experience with PICC lines/dressing changes, trachs, port access, ostomies, chest tubes, Pleurex drains, S/p catheters, lab draws/UA/C&S, etc.
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 8a-4:30p, Mostly M-F with rotating weekend every third weekend
