Schedule: Day Shift, 5×8 Hours (8:00 AM–4:30 PM)
Requirements:
- Minimum 2 years of acute care hospital Case Management experience
- Active California RN License (pending license accepted if active by start date)
- Epic experience required
- Previous travel experience preferred; first-time travelers accepted
- Experience in Care Coordination and Discharge Planning
- Knowledge of InterQual criteria preferred
Additional Details:
- Patient Ratio: 1:20
- No floating required
- Scrubs or business attire accepted
- Experience arranging DME, SNF placement, and ambulance transportation required
