Job Description
We are seeking an experienced Registered Nurse (RN) Case Manager for a 26-week travel in Mission Hills, California. The ideal candidate will have at least 3 years of acute-care hospital case management experience, with a strong background in care coordination, discharge planning, and utilization review.

Required Qualifications

• Active California RN license (pending license accepted, but must be active by start date).
• Minimum 3 years of acute-care hospital Case Management experience.
• Previous experience in care coordination and discharge planning.
• Recent experience with Epic charting.
• Weekend REQ: weekend rotation + meet dept needs
• HOSPITAL EXPERIENCE IS A MUST THEY NEED TO HAVE
