POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Responsible for providing coordination of care for patients to support safe, seamless, timely transitions across the continuum.?This role utilizes a collaborative process, the RN Care Manager identifies (using quantitative and qualitative methods), assesses, plans, implements and evaluates the options and services required to meet an individual’s health and health related needs, including social- determinants that affect ones’ overall wellbeing.?The RN Care Manager is responsible for screening, identification, and assessment of individuals in need of active Case Management services and promotes the right resources, at the right time and at the right place.

MINIMUM REQUIRED QUALIFICATIONS –

• VA/compact RN license
• Associate degree in Nursing
• AHA BLS
• 1+ year acute care hospital experience (will consider IRF or LTAC but prefer acute care experience)

PREFERRED QUALIFICATIONS –

• Bachelor of Science in Nursing
• ACM or CCM
• 3 years discharge planning
• Mother/Baby experience
• EPIC, CarePort Parachute experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Monday-Friday 8a-4:30p with one weekend per month

SYSTEMS – EPIC, CarePort Parachute

START DATE – 11/30/2026
