Hospice RN - Case Manager Full Time

Trustbridge Hospice , a member of Empath Health , is seeking a compassionate and dedicated RN Hospice Case Manager to join our team serving patients and families across Southern Palm Beach County. Your schedule would be: 8:30am - 5pm & every other weekend. Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care. As an RN Case Manager, you will play a vital role in coordinating care and supporting patients and families throughout their hospice journey. Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team Develop and implement individualized plans of care as part of the IDG (Interdisciplinary Group) Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life Why Join Empath Health? Registered Nurse currently licensed in the State of Florida. One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred. Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs Basic computer skills and EMR software experience a plus.
