Job description
Empath Home Health , a member of Empath Health , is seeking a compassionate full-time Home Health RN to work with our Home Health team to deliver individualized care to patients in their homes in the Ocala area (Marion, Lake, & Sumter counties). Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.
Locations: Home health visits in Ocala area (Marion, Lake, and Sumter counties)
Schedule: Full time; M-F 40 hours with required weekend/ holiday on-call rotations
Develop, initiate, and revise individualized plans of care as patient needs evolve
Deliver skilled nursing services and treatments as outlined in the plan of care
Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
Collaborate with the interdisciplinary team to ensure seamless, holistic care

Graduate of an approved nursing program with an active Florida RN license
Minimum of 1 year of direct patient care experience; prior home health RN experience strongly preferred
Willingness to participate in weekend and holiday on-call rotations
