Hospice Admission/Liaison Registered Nurse

Tufts Medical Center seeks a compassionate Hospice Admission/Liaison Registered Nurse to guide patients and families through the hospice admission process and coordinate seamless transitions of care. In this field-based role, you will conduct comprehensive assessments, explain hospice services, obtain consents, and develop initial care plans aligned with patients' goals.
