Description
As a Registered Nurse Case Manager, you will coordinate patient care and ensure that each patient receives the highest quality of service. You will work closely with healthcare teams in a fast-paced acute care setting, managing cases throughout the continuum of care. Your role is crucial in facilitating smooth transitions and optimizing patient outcomes. A strong focus on communication and collaboration is essential, and you will be expected to adapt to various situations, including floating to different units as needed.
