Job Summary
We are seeking an experienced Registered Nurse (RN) to provide skilled nursing care to patients in their homes. The RN will assess patients' health conditions, develop and implement individualized care plans, monitor patient progress, and coordinate care with physicians and other healthcare professionals.
Responsibilities
- Conduct patient assessments and monitor health conditions.
- Develop, implement, and evaluate individualized patient care plans.
- Perform wound care and manage Wound VAC therapy.
- Complete OASIS assessments and maintain accurate patient documentation.
Requirements
- Active Georgia RN license or Compact RN license.
- Previous Home Health nursing experience.
- Experience with HCHB (Homecare Homebase).
- OASIS assessment experience.
