Shift: 8:00 AM - 4:00 PM
Schedule: Rotating weekends: Every 4th or 5th weekend Holidays: One summer and one winter holiday
Duration: 13 Weeks
Job Summary:
- Coordinates patients through the acute care hospitalization Manages all aspects of discharge planning/transition process
- Performs utilization management and quality screenings
Requirement:
- 2 years of acute care experience strongly preferred
- Case Management, Managed Care. In Patient Case Management is required
- Hospital experience required, including experience with discharge planning and placement in rehabilitation facilities
- Minimum 2 years of experience with acute inpatient hospital discharge planning
- Experience with subacute patients required
Pay Rate is negotiable
