Job Summary

Facility is seeking an experienced Registered Nurse (RN) Case Manager with a strong background in acute-care hospital settings. The ideal candidate will be responsible for coordinating patient care, managing utilization reviews, facilitating discharge planning, and ensuring patients receive appropriate and cost-effective care throughout their hospital stay.

Key Responsibilities

• Coordinate and manage patient care throughout the acute-care hospitalization process.
• Conduct patient assessments and evaluate medical, psychosocial, and discharge needs.
• Develop and implement individualized discharge plans.
• Perform utilization reviews to ensure medical necessity and appropriate resource utilization.
• Collaborate with physicians, nursing staff, social workers, and multidisciplinary healthcare teams.
• Coordinate with insurance companies and third-party payers for authorization and continued-stay reviews.
• Facilitate transitions of care to home health, skilled nursing facilities (SNF), rehabilitation centers, and other post-acute settings.
• Monitor patient progress and identify barriers to timely discharge.
• Maintain accurate documentation of case management activities and patient care plans.

Required Qualifications

• Active Registered Nurse (RN) license and BLS from AHA.
• Minimum 4 years of RN Case Management experience.
• Previous experience in an acute-care hospital setting.
• Strong knowledge of utilization review and discharge planning.
• Excellent communication, organizational, and critical-thinking skills.
