Job Summary
Long Term Care position providing post-acute skilled nursing services for residents recovering from surgery or illness. The role supports care related to chronic conditions, wound care, tracheostomy care, nasogastric and gastrostomy tubes, mobility assistance, activities of daily living, rehabilitation, hospice, and end-of-life needs.
Job Details
- Schedule: Day shifts are 12 hours, with a minimum of 36 hours per week.
- Required Qualifications: Basic Life Support (BLS) Certification and a minimum of 2 years of experience.
- Additional Information:
- Provides skilled nursing care in a post-acute setting for residents with post-surgical conditions and chronic diseases, including chronic obstructive pulmonary disease, congestive heart failure, diabetes, renal failure, cerebrovascular accidents, pneumonia, sepsis, joint replacement recovery, and cancer.
- Responsibilities may include wound care, tracheostomy care, nasogastric tube care, gastrostomy tube care, mobility support, activities of daily living assistance, and coordination of recovery-focused care.
- Supports residents transitioning toward greater independence and discharge to the community.
- Provides hospice and end-of-life care, including education and support for families.
