🏠 Home Care / Geriatric Focus
Position Highlights:
- Provide direct nursing care to frail, elderly, and long-term care participants
- Perform home visits for wound care, lab draws, assessments, treatments, and other nursing services
- Monitor acute and chronic medical conditions
- Administer medications, immunizations, and prescribed treatments
- Participate in care planning and coordinate with interdisciplinary care teams
- Perform triage of emergent situations and manage complex clinical needs
- On-call needs and weekend/holiday work may be required
Requirements:
- Associate Degree in Nursing Required | BSN Preferred
- RN Nursing Diploma/Degree from an Accredited School
- 3+ Year Experience with Frail Elderly, Long-Term Care, or Similar Population
- Home care experience preferred
- Wound care experience preferred
- Current AHA CPR Certification or willingness to obtain
- Current Michigan RN License
- Valid Michigan Driver’s License & Vehicle Insurance
- Reliable Transportation Required
- Current required immunizations or appropriate medical documentation
