Requirements:
- 2+ years Acute Care Case Management/Discharge Planning experience
- Hospital Case Management experience required
- Behavioral Health experience alone does not qualify
- Experienced traveler required - no first-time travelers
- Active CA RN license
- Experience with care coordination, discharge planning & utilization review
- Knowledge of HMOs, IPAs, CMS, DRGs & Joint Commission standards
- Must independently arrange DME, SNF placements & ambulance transport
Additional:
- Patient ratio: 1:25 (weekends up to 1:45)
- Every other weekend as needed
- No floating
- Acute care facility
- Adults, geriatrics, adolescents & neonates
- Business attire or scrubs
- Maximum 7 days RTO
