Job Description
We are seeking an experienced Case Manager to support the healthcare team in coordinating patient care, facilitating discharge planning, and ensuring effective utilization of healthcare resources.
Responsibilities
Coordinate patient care and collaborate with physicians, nurses, and interdisciplinary teams.
Assess patient needs and develop individualized care plans.
Facilitate discharge planning and ensure smooth transitions of care.
Monitor patient progress and identify barriers to care.
Support utilization management and ensure appropriate use of healthcare resources.
Maintain accurate patient records and documentation.
Communicate with patients, families, and healthcare providers regarding care plans and follow-up needs.
Requirements
Relevant experience in case management within a healthcare setting.
Current BLS certification.
Strong communication, organizational, and care coordination skills.
Ability to work collaboratively with multidisciplinary teams.
