15,000 Sign-On Bonus*** Job Title: Oncology Nurse Navigator West Cancer Center is a comprehensive oncology practice where compassionate care meets innovative medicine. The Oncology Nurse Navigator serves as a dedicated care coordinator throughout the cancer care continuum. This role assesses and addresses the physical, emotional, and social needs of patients, while working closely with healthcare providers and internal/external partners to remove barriers to care. Assess, plan, facilitate, and coordinate care to meet patients’ physical, emotional, and social needs Advocate for timely, high-quality care by guiding patients through the healthcare system Coordinate appointments, referrals, and services to ensure continuous care Provide education on treatment plans, symptom management, and health maintenance Reduce barriers to care through anticipatory guidance, insurance coordination, and community resource navigation Collaborate with oncologists, specialists, and support teams to enhance patient experience and outcomes Educate patients on individualized care plans to reduce unnecessary utilization and improve quality of life Serve as a trusted resource for patients and families throughout the cancer journey Perform RN scope duties, including telephone triage and clinical assessments Participate in clinic activities and assist in care coordination and planning Graduate of an accredited nursing program (required) At least 1 year of nursing experience (required) Bachelor’s degree in nursing (preferred) Outpatient nursing and scheduling experience (preferred) Current RN license in the state of practice or compact license (required) Strong knowledge of oncology practices and patient navigation Work alongside a multidisciplinary team of dedicated professionals committed to improving cancer care.
